Make a Memorial Tribute Gift

At a difficult time in our lives, we often think of making Memorial or Tribute Gifts as gestures of love, remembrance and appreciation in lieu of sending flowers. The Garnet Health Foundation receives many Memorial Gifts throughout the year, and often they are designated for a specific area or department of the hospital. The Foundation Office sends an acknowledgement to the family that a gift was made in memory of their loved one. If you would like to direct family and friends to make memorial gifts to Garnet Health Medical Center, the following is suggested language for an obituary or other tribute:

In lieu of flowers, gifts may be made to Garnet Health Foundation in memory of {deceased name} to Garnet Health Foundation, 707 East Main Street, Middletown, NY 10940. If you would like to make a Memorial Gift online, please utilize the form below and indicate in the Tribute Gift section.
