Case Management Services

As a patient at Garnet Health a case manager will be assigned to assist you and family members with discharge planning to ensure a safe return home. The case manager will familiarize and educate you on available community resources. Family conferences are arranged through the case manager to discuss your goals and progress. Concerns regarding financial assistance and/or insurance benefits are addressed by the case manager.

The case manager can help you in a variety of ways, including:

Post Hospital Planning

By working with your doctor and other health providers to successfully prepare for your discharge and any necessary after-care. This could include home-care assistance, public health services or placement in a short-term (rehabilitation) or long-term care (nursing home) facility.

Problem Solving

By helping patients and their loved ones deal with the impact of illness on their daily lives. This department provides information concerning community healthcare services and facilities supportive and protective services.
