Who needs the Short-Term Rehabilitation Program?
The person who:
- Is recovering from major surgery, including joint replacement
- Has experienced repeated hospitalizations
- Is recovering from a stroke
- Is recuperating from a major accident
- Needs help managing pain
- Requires wound care or amputation care
- Is coping with congestive heart failure
- Is gradually getting better from a serious condition
- Needs enteral nutrition
To qualify for the Short-Term Rehabilitation Program, patient must:
- Be under the supervision of professional personnel
- Require skilled services on a daily basis
- Have a 3-day acute admission prior to entering the Short-Term Rehabilitation Program
