The Garnet Health Medical Center Patient Transfer Center includes one-call customer service, operating 24-hours a day, seven days a week to facilitate urgent transfers for patients age 18 and older to our Hospital.

Transfer Center at Garnet Health

We assist all parties involved with the transfer of urgent adult and Emergency Department admissions to Garnet Health Medical Center. Our goals are to streamline the referral and acceptance process, making it less time consuming and more convenient for everyone.

The Garnet Health Medical Center Transfer Center staff quickly connects referring physicians and secures the appropriate bed. Whether a patient is in another hospital, emergency department or medical office, the process is simple:

Step 1:

The referring physician calls 844-676-2233

Step 2:

The Transfer Center's staff collects the following information from the referring physician:

Your name
Referring physician and/or hospital
Your phone number
Patient's name
Patient's condition- age, diagnosis, level of care needed and type of specialty care or service requested
Patient's primary care provider (PCP) name and phone number

Step 3:

The Garnet Health Medical Center Transfer Center will:

Contact the requested service on call physician
Facilitate physician connection for patient report

If the patient is accepted, the transfer center will:

Assist with transportation arrangements if required
Coordinate nursing unit and bed arrangements for the patient
Facilitate the transmission of necessary paperwork and medical documents from your hospital to Garnet Health Medical Center
Notify you once the patient arrives at the hospital

We also will provide referrals and other information for continuum of care, if your patient requires services not offered at your facility.
