Community Service Plan, Community Health Needs Assessment & Community Health Improvement Plan
Each year, Garnet Health Medical Center prepares a Community Service Plan, a Community Health Needs Assessment and Community Health Improvement Plan, as required by the New York State Department of Health. The plans describe the hospital’s mission and vision, as well as specific measures it is taking to improve the health and well-being of the community.
Such as in past years, we have developed an implementation strategy to illustrate the hospital’s specific programs and resources that support ongoing efforts to address the identified community health priorities. This work complements objectives of the local health department and is supported by community-wide efforts and hospital leadership.
